Coronavirus: If you suspect that you have Coronavirus please DO NOT visit the surgery. Please firstly read the advice published by Public Health NI. A link to their website is as follows https://www.publichealth.hscni.net/news/covid-19-coronavirus If you still have concerns, are symptomatic and have travelled to one of the high risk areas please contact us by telephone on the usual number or if you have returned from one of the listed countries (as per Public Health) and are asymptomatic or just want advice please contact 111 - this service is available 24 hours a day.

Please also only visit our treatment room/bloods room if you really need to.

We are now able to contact our patients by SMS Text message. If you would like to sign up for this service please complete and return a consent form. Consent forms are attached SMS Text consent form or alternatively you can get one from reception.

Self referral to physiotherapy

If you are registered with a GP in the South Eastern Trust area and you are over 16 years old, you can now refer yourself to Physiotherapy.

Suitable for those suffering from low back pain, neck pain, or recent injuries such as strains and sprains , or joint and muscular pain.

A direct route to expert physiotherapy advice and treatment

Promotes your ability to manage your problem and prevent it getting worse or returning

3 ways to access a referral form:

1. Online form - A form can be completed online and submitted at the following link Online Physio Self Referral Form

2. PDF form - forms can be downloaded and printed from the South Eastern Trust website Physiotherapy page

3. Paper form - a copy of the form can be picked up at our reception or South Eastern Trust Physiotheraphy Departments. Completed forms can be posted or emailed to the South Eastern Trust AHP Central Booking Office: ahp.centralbookingoffice@setrust.hscni.net
